Accident on Hwy. 21 on Aug. 12 claims a life, leaves two injured

At 12:59 p.m. on August 12, a crash occurred on WIS 21 EB at CTH V in Waushara County near Coloma.

Assisting agencies included Waushara County Sheriff’s Office, Adams County Sheriff’s Office, DNR, EMS, Coloma Fire Department, and Waushara Highway Department.

The crash involved one fatality and two injuries. Kevin T. Butler, 65, of Sturgeon Bay, the driver of the SUV was pronounced dead at the scene. The only other passenger, Carol T. Butler, 63, of Sturgeon Bay, was flown to the UW-Madison Hospital with serious injuries. The semi driver, Tomasz Dubial, 34, of Bloomington, IL, was also flown to UW-Madison with unknown injuries.

 

Wisconsin State Patrol and Waushara County Sheriff’s Office, responded to the semi verses car crash. The semi was traveling southbound on CTH V, never slowed, continued through the stop sign at the intersection of CTH V and STH 21 at normal highway speeds, then crashed into an SUV traveling eastbound on STH 21.

The State Patrol is invest-igating the crash.
