Waushara County Health Department has received a limited supply of the new COVID-19 vaccine. Due to the commercialization of the vaccine please review the below eligibility changes from previous doses:
Those able to receive the COVID-19 vaccine at no out-of-pocket cost.
65 and older on Medicare, or
65 and older with a Medicare Supplement, or
On Medicaid (Forward Health), or
Any age with no health insurance
Those on Medicaid, Medicare or with a Medicare Supplement, MUST BRING insurance cards to their appointment to receive the vaccination.
Those able to receive the COVID-19 vaccine with payment due at the time of service.
Those who have a Commercial (Private) Health Insurance Policy
Insurance purchased on the Marketplace.
Insurance provided by an Employer.
The Health Department is unable to bill Commercial (Private) Health Insurance. Due to this, you will be required to pay for the cost of the vaccination. Payment can be made via cash or check. For payments made via credit or debit card there will be an additional processing fee of 3.95%
